The personal data processor is: EVERTILE INTERNATIONAL s.r.o., with its registered office at Bezručova 1601/84, Říčany near Prague.

Any employee, partner, or external contractor of the controller is also authorized to process personal data.

TEXT OF THE NOTICE

In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter also “GDPR”), the following information is hereby provided to you, as data subjects, specifically regarding (1) what personal data we collect, (2) how we handle this data, (3) for what purposes we use personal data, (4) to whom we are authorized to disclose this personal data, (5) what your rights are regarding personal data protection, as well as (6) where you can obtain information about your personal data that we process. Your personal data is always handled in accordance with applicable laws to ensure its security to the greatest extent possible.

In cases where you wish to exercise these rights against the Controller, it is necessary to contact the Controller at the email address hascynova@evertile.net . Each such request will be evaluated by the Controller, and the data subject will be informed of the result of this evaluation by the Controller within 30 days of the Controller’s receipt of such a request.

FINAL PROVISIONS

As a data subject, I declare that I am aware of my rights under Sections 12 and 21 of the Personal Data Protection Act and my rights under the GDPR.

I declare that all data provided is accurate and truthful and is provided voluntarily.

The controller declares that it will collect personal data to the extent necessary to fulfill the purpose and will process it only in accordance with the purpose for which it was collected.
